For people with nearsightedness, clear daytime vision usually means wearing glasses or contact lenses. Orthokeratology, commonly called Ortho-K, takes a different approach. These specialized contact lenses are worn while you sleep and gently reshape the surface of the cornea so you can see more clearly during the day without corrective lenses. Ortho-K lenses are a terrific option for patients who have contact lens comfort issues.
The cornea is the clear front surface of the eye that helps focus incoming light. When someone is nearsighted, light focuses in front of the retina instead of directly on it, causing distant objects to appear blurry.
Ortho-K lenses are specially designed rigid contact lenses that apply controlled forces to the tear layer and corneal surface while the eyes are closed. Overnight, the fluid within cells making up the outermost layer of the cornea redistributes slightly. The central cornea becomes flatter while the surrounding area changes shape in a carefully controlled pattern.
This temporary change adjusts how light is focused inside the eye. After removing the lenses in the morning, many patients can experience clear functional vision throughout the day.
Unlike regular contact lenses that correct vision while they are being worn, Ortho-K lenses create a temporary corneal shape change that continues to provide vision correction after the lenses are removed.
The process generally involves:
• Wearing customized Ortho-K lenses overnight as directed
• Allowing the lenses to gently reshape the corneal surface during sleep
• Removing the lenses in the morning for lens-free daytime vision
• Wearing the lenses consistently to maintain the desired correction
• Attending follow-up appointments so we can monitor vision, corneal health, and lens fit
Because the effect is temporary, the cornea gradually returns toward its original shape if Ortho-K wear is stopped.
Ortho-K is especially valuable for children with progressive myopia. In addition to correcting blurry distance vision, the optical changes created by these specialized contact lenses can help influence how light reaches the peripheral retina. Orthokeratology can help slow axial eye growth and myopia progression in many children.
Slowing myopia progression matters because higher levels of nearsightedness are associated with an increased lifetime risk of certain eye health problems. We consider factors such as a child's current prescription, rate of myopia progression, family history, visual habits, contact lens tolerance, and overall eye health when recommending a treatment strategy.
Precise measurements of the eyes and corneal shape are necessary to design lenses that provide the intended correction while maintaining healthy corneas. Follow-up care is equally important. As vision and eye shape change, we can monitor the results and determine whether adjustments are necessary. Our vast experience with specialty contact lenses allows us to create individualized solutions based on each patient's visual needs.
